ABSTRACT

ObjectiveTo develop a deep learning system for early ultrasound screening of developmental dysplasia of the hip (DDH), a new smart-hip ultrasound technique (S-hip), and to validate its clinical application. MethodsWe selected 11,100 annotated and reviewed coronal ultrasound images of infant hips between November 2021 and August 2022, 8,100 of which were used for the training set and 3,000 for the test set, to build a S-hip deep learning system. To verify the consistency between the automated measurement by S-hip and the manual measurements by sonographers, 174 standard coronal ultrasound images of 87 infants' bilateral hips were acquired, then α angle, β angle and femoral head coverage (FHC) were measured by S-hip, an ultrasound expert and a resident. The measurement data and the time required for the measurements were recorded and statistically analyzed. Another 100 standard coronal ultrasound images of the hips were randomly selected and measured twice respectively by the ultrasound expert and resident to assess the intra-sonographer repeatability. ResultsThe intraclass correlation coefficient (ICC) (95% CI) values of α angle, β angle and FHC results measured by S-hip and ultrasound expert were 0.799 (0.738, 0.847), 0.798 (0.737, 0.846) and 0.934 (0.954, 0.975), respectively. Those values measured by the ultrasound expert and resident were 0.725 (0.645, 0.789), 0.674 (0.583, 0.748) and 0.931 (0.908, 0.949), respectively. The mean absolute errors (MAE) of α angle, β angle and FHC results between measurements by S-hip and ultrasound expert were 2.69 °, 4.43 ° and 2.47%, respectively. The time required for measurements by S-hip, ultrasound expert and resident was (1.59±0.36) s, (18.76±2.23) s and (19.45±2.76) s, respectively. The automated measurement by S-hip cost much shorter time than the manual measurements by sonographers and the difference was statistically significant (P<0.001). The ICC (95% CI) values of α angle, β angle and FHC results between two measurements by the ultrasound expert were 0.943 (0.916, 0.961), 0.959 (0.940, 0.972), and 0.981 (0.971, 0.987), respectively. Those values by the ultrasound resident were 0.884 (0.833, 0.921), 0.921 (0.884, 0.946), and 0.962 (0.944, 0.974). ConclusionThe S-hip based on a deep learning system is a highly reliable automated technique to accurately measure α angle, β angle and FHC. Compared with ultrasound residents, S-hip allows for a more simplified and significantly quicker measurement, which may enhance the widespread use of hip ultrasound screening in infants.

ABSTRACT

Objective:To observe the effect of early weight-bearing on the appropriate population with intertrochanteric fracture after surgery. Methods:From April, 2017 to December, 2018, a total of 45 patients with Evans-Jensen type II intertrochanteric fracture and fracture reduction as positive medial cortex support (PMCS) after proximal femoral nail anti-rotation (PFNA) fixation were randomly divided into control group (n = 22) and experimental group (n = 23). Weight-bearing as tolerated (WBAT) was initiated from six weeks after surgery in the control group, and within 48 h after surgery in the experimental group. The frequency of WBAT in two groups increased gradually from three times a day for ten minutes a time to five times a day for 20 minutes a time until clinical healing of fracture. The length of stay, hospital cost, the fracture healing time and the complication incidence were compared between two groups, as well as the scores of Visual Analogue Scale (VAS) and Harris Hip Score at six weeks, three months and six months after surgery. Results:Compared with the control group, the length of stay was shorter (t = 3.716, P < 0.01), the hospital cost was lower, but no significant difference was found (t = 1.540, P > 0.05), and the fracture healing time was shorter (t = 6.248, P < 0.001) in the experimental group. The complication incidence was lower in the experimental group, but there was no significant difference (χ2= 2.198, P > 0.05). Six weeks, three months and six months after surgery, there was no significant difference in the score of VAS between two groups (t < 1.330, P > 0.05). The score of Harris Hip Score was significantly higher in the experimental group than in the control group six weeks after surgery (t = -5.115, P < 0.001), however, no significant difference was found in other time points (|t| < 1.799, P > 0.05). Conclusion:Early weight-bearing within 48 h after PFNA fixation for Evans-Jensen type II intertrochanteric fractures and reduction with PMCS could shorten the length of stay, shorten the bony healing time and promote early recovery of hip function.

ABSTRACT

Hepatocyte growth factor (HGF) is a multifunctional cytokine that is related to many diseases. HGF mainly contributes to cell migration,proliferation,and survival and regulates vascular angiogenesis,matrix deposition,and degradation of wound healing. HGF also promotes wound reepithelialization and reduces scar formation. This review article summarizes the role of HGF in wound repair and the relationship between HGF and other growth factors,especially when applied for the clinical treatment of chronic skin ulcers.

OBJECTIVE@#To discuss the mechanism of low molecular weight GTP binding protein RAC1 in the injury of neural function based on building the rat model of cerebral ischemia reperfusion.@*METHODS@#Middle cerebral artery of rats was ligated and the ligature was released to restore the perfusion after 2 h, the rat model of cerebral ischemia reperfusion injury was built, while the middle cerebral artery was ligated. The rats were randomly divided into the sham group, cerebral ischemia reperfusion group (I/R group) and the group with the injection of RAC1 activity inhibitor NSC23766 (NSC group). The survival and neurological severity score of rats in each group were observed and recorded. Nissl staining was employed to observe the nerve cells, and Western blot to detect expression of RAC1, superoxide dismutase and malondialdehyde.@*RESULTS@#Number of nerve cells for rats in NSC group was significantly more than that in I/R group, but significantly less than that in sham group, with the statistical difference (P < 0.05). The brain water content for rats in NSC group was significantly lower than that in I/R group, but significantly higher than that in sham group, with the statistical difference (P < 0.05). The expression of RAC1 and malondialdehyde for rats in NSC group was significantly lower than that in I/R group, but higher than that in sham group; while the expression of superoxide dismutase was lower than that in sham group, but higher than that in I/R group, with the statistical difference (P < 0.05).@*CONCLUSIONS@#The inhibition of RAC1 activity can reduce the oxidative stress, reduce the neurologic impairment because of cerebral ischemia reperfusion and thus protect the neural function.

ABSTRACT

To observe the reliability and short-term and mid-term efficacy of a new designed multi-functional lacrimal sac nasal drainage stent on the treatment of nasolacrimal duct obstruction after endonasal endoscopic dacryocystorhinostomy ( EES-DCR ) , and to evaluate its clinical application value. METHODS: A retrospective case series study was carried out. Seventy - six patients ( 79 eyes ) with nasolacrimal duct obstruction underwent EES-DCR and a new type of multi-functional lacrimal sac nasal drainage stent was implanted. The nasal lacrimal drainage stent was pulled out 2~3mo later after surgery and the patients received 3~6mo follow-up afterwards. RESULTS: ln 79 eyes, stents were successfully implanted in 77 eyes (97. 5%). All stents of 77 eyes could be pulled out successfully 2 ~ 3mo after the operation. Sixty eyes were cured with unobstructed lacrimal irrigation and symptom of epiphora disappeared;15 eyes were improved of epiphora and mild reflux during lacrimal irrigation were found, overall effective rate was 97. 4%, 2 eyes failed in stent implantation. ln 6mo follow-up, nocomplications such as stent displacement or slip was occurred, the epithelization of anastomotic stoma was good.CONCLUSlON: Surgical technique of using the multi-function nasolacrimal stent is simple and fast, and implantation of the new designed stents has a rapid recovery with fewer complications. The usage of the new stents not only improves the success rate but also broadens surgical indications of EES-DCR, it is a safe and effective method for the treatment of nasolacrimal duct obstruction.

ABSTRACT

Objective To establish a 3D finite element model of fixed denture for the preparation of the subsequent study such as biomechanical analysis. Method The computed temography(CT) images were processed by Mimics and Geomagic studio,then the solid model and finite model were established by UG NX. Results A 3D finite element model of fixed denture including alveolar bone, abutment, pericementum and denture were established. Conclusions The use of the initial data of teeth to build finite model can avoid data loss and minimize model distortion to a certain extent, so the 3D finite element model has good similarities in geometry.
